چکیده

The pituitary gland has only recently been studied with magnetic resonance (MR) imaging. Initial results suggest MR may have utility in the detection and follow-up of pituitary microadenomas [1]. Of interest, it has been observed that prolactinomas treated with bromocriptine may manifest distinct and diverse MR signal changes [1] . Bromocriptine, a dopamine agonist, has been effective in the symptomatic treatment of many prolactinand in rare instances growth-hormone-secreting tumors, causing reduction in tumor size and normalization of elevated serum hormonal levels. The apparent MR signal changes that have been observed thus far using spin-echo pulse sequencing are increased signal intensity on T1-weighted sequences suggesting shortening of T1 relaxation time and both increased or decreased signal intensity on T2-weighted sequences suggesting, respectively, prolongation or shortening of T2 relaxation time. Initial surveys suggest that about onethird of prolactinomas clinically responsive to bromocriptine manifest MR signal changes (Bradley WG Jr, personal communication). I wish to propose an explanation of this phenomenon that considers histologic changes observed in prolactinomas responsive to bromocriptine therapy. I also wish to speculate on possible implications these MR signal changes may have in the initial diagnosis and management of prolactinand possibly growth-hormone-secreting pituitary adenomas. Several histologic changes are observed in prolactinomas after short-term (6 weeks) bromocriptine administration [2 , 31: (1) marked reduction in tumor cell size , primarily because

Pituitary adenomas: early postoperative MR imaging after transsphenoidal resection.

BACKGROUND AND PURPOSE Although there have been several reports on postoperative MR imaging of the sella, immediate postoperative changes (usually within 3 days) have not been extensively analyzed. The purpose of this study was to establish the value of early postoperative MR imaging in differentiating residual tumor from postoperative surgical changes in the sella after transsphenoidal resecti...

Aspects of medical therapy of neuroendocrine disorders.

The effects of long term in vitro exposure witil octreotide of human GH secreting pituitary adenoma cells were investigated on GH release, intracellular GH concentrations, and GH mRNA levels. Incubation of tile adenoma cells for 4 days witil 10nM octreotide induced a dose-dependent inhibition of GH release, while in parallel an increase (maximal increase varying between 24 and 517%) in the intr...

Long-term octreotide therapy in growth hormone-secreting pituitary adenomas: evaluation with serial MR.

PURPOSE To compare the changes in tumor volume with length of octreotide treatment in patients with acromegaly, to analyze signal alterations of the pituitary mass during treatment, and to determine an optimal MR imaging protocol. METHODS Eighteen patients with growth hormone (GH)-secreting pituitary adenomas were studied with MR imaging before and during octreotide treatment. The length of f...

Effects of Thyroid Hormone (T4, T3 ), Bromocriptine And Triac on Inappropriate TSH Hypersecretion

Inappropriate TSH hypersecretion was diagnosed in a 34 years old woman. Earlier she had been treated by ablative therapy for hyperthyroidism. The present diagnosis was based on elevated basal serum TSH levels despite elevated serum free thyroid hormone level. Her TSH responses to TRH (peak value 240 mu/I) was high. Her albumin and prealbumin levels were normal. The serum TBG level, alpha subuni...
